To add additional pages to your passport please follow the simple instructions below and contact us so we can begin the process in our system and review all the procedures to insure superior customer service.

Please prepare the following documents:
 
1 - Passport DS-4085 Application – Must be completed online and downloaded from the Government Website. Please print, fill out and sign.

2 - Check or Money Order for $142.00 – Made out to: The U.S. Dept. of State

3 - Signed Letter of Authorization – Must sign and have on top of paperwork. Download Here.

4 - Your Current Passport – These must be valid passports. 

5 - Itinerary – A copy of your Flight Plan. If you have not scheduled your travel plans yet please let us know.

Once these documents are ready please send to our office for expedited processing.
