To replace a lost or stolen passport please follow the simple instructions below and  contact us so we can begin the process in our system and review all the procedures to insure superior customer service.

Please prepare the following documents:

Note:These documents need to be 'Executed & Sealed' at the nearest Post Office ($25.00) or Clerk of Court, then we are able to expedite the processing for you.
 
1 - Passport Application DS-11 & DS-64: Statement Regarding Lost or Stolen Passport – Must be completed online and downloaded from the Government Website. Print Application and fill out. Do Not Sign Must sign at the Post Office or Clerk of Court.

2 - Check or Money Order for $170.00 – Made out to: The U.S. Dept. of State

3 - Original Birth Certificate – This will be returned with your Passport.

4 - Signed Letter of Authorization – Must sign and have on top of paperwork. Download Here.

5 - Copy of ID or Driver’s License – Include as second page.

6 - Two Passport Photos – Taken within the last 3 months. This can be the same picture that is in the passport.

7 - Itinerary – A copy of your Flight Plan. If you have not scheduled your travel plans yet please let us know.




Once these documents have been Executed & Sealed send to our office for expedited processing.
